Petition For Adoption
If you are filing a Petition For Adoption, please note:
- All records of Adoption are confidential and exempt from the provisions of Section 119.07 (1) Florida Statutes, except as provided in Section 63.162 F.S.
- Subject to inspection only upon Order of Court, the Clerk and Comptroller's office cannot furnish information pertaining to Adoptions to customers. This includes the Attorney of Record and/or the petitioners.
- Pursuant to Section 63.162 F.S., a Court Order is required allowing access to the Adoption file.
Certified Statement of Final Judgment of Adoption
Along with the filing of the Petition for Adoption, you must complete parts A and B of the Certified Statement of Final Judgment of Adoption (Department of Health, Vital Statistics) and file it with the clerk. The Clerk and Comptroller will forward both to the State of Florida, Department of Heath, Vital Statistics; or you may mail a copy of the Certified Statement of Final Judgment along with payment to:
